Slough man took own life at Langley station, inquest hears

An inquest into the death of a 32-year-old man from Slough was held at Reading Town Hall yesterday (Thursday).

Coroner Peter Bedford concluded that Daniel Philip Adams, who lived in Jackson Close, took his own life while suffering from depression.

The inquest heard that Mr Adams was hit by a high speed train at Langley Railway Station on Monday, February 8.

Mr Bedford said he was at high risk of suicide due to the breakdown of his marriage, and he had told his therapist that he did not want a life without his wife and child.

Mr Adams had received treatment at Prospect Park from October to November and he had seen a therapist four days before his death.

The inquest heard from a cognitive behaviour therapist who had expressed concern for Mr Adams’ safety. She had attempted to refer him to the crisis team, however this was declined due to the significant level of risk not being ‘fully appreciated’.

Concluding the case, Mr Beford said this was a ‘missed opportunity in proactive intervention’ by the crisis team.

Mr Bedford did note that Mr Adams had sent an email to his therapist confirming his attendance for a session for the following day on day he died which gave ‘a false sense of reassurance’.
